The Coda file system meets half your requirements ;-)

It's also really bad C++.  Check it out for what not to do, on average.

Why not look at the GNU class library or libstdc++?  Although the
library as a whole is large and complex, the individual modules
probably come from bite-size to Michelin 3-star five course meals.

Bjarne's book (first edition) is good, with a fair number of examples.
